As the contestant or parents/legal guardian of the contestant named above, I do hereby agree to abide by the rules and regulations as set forth by the Miss Elite Image of America Pageant. I agree to hold Miss Elite Image of America Pageant staff members, and volunteers harmless of any damages or liabilities due to theft, accident or injury during or resulting from any contestant’s participation in this pageant. I understand and agree that in the case of cancellation of the pageant, that said pageant will be canceled without benefit of refund; however, the pageant will be rescheduled at another time and location. I understand that the judges’ decisions are final. I understand that failure to show good sportsmanship may result in disqualification of the contestant without refund and forfeiture of any awards received. I fully give Miss Elite Image of America Pageant permission to use photograph(s) of me for their promotional use and I understand I will not receive any compensation for use of my photograph(s). I agree and confirm that all of the enclosed information is true and correct to the best of my knowledge and belief. I understand and agree that the above named contestant, if declared winner will return at the end of her reign to crown her successor. Miss Elite Image of America has the right to accept or deny any application at their discretion. If chosen as the Miss Supreme Elite Image of America winner, I understand that I will need to sign the Miss Supreme Elite Image of America contract that will be provided at the end of the pageant to receive any and all prizes. I understand that failure to sign the Miss Supreme Elite Image of America contract provided will result in me forfeiting the Miss Supreme Elite Image of America title and all prizes. Prizes are subject to change. Contestants will not hold Miss Elite Image of America, Pageant Image Consulting Inc., or its affiliates responsible for any travel fee or hotel accommodations to compete at our pageant. Miss Elite Image of America and its affiliates are not responsible for airfare cost or required travel documents for the Supreme Elite Image of America winner. By submitting this application, I understand that all payments are non-refundable and non-transferable. Unless I have received a formal payment extension from the pageant office, I understand that payment is due at the time I submit my application. I understand that each payment will incur a $5 service fee that is non-refundable and non-transferable. Signature below is required for your application to be complete.
